On my huge map game I managed to have voidworm crisis if all 3 of the nests, each on its own black hole starting zone, are still alive.
More problematic if you got OP mods installed that give too much firepower in the early game, causing AI empires to be powerful enough to start eradicating void worm nests.
maybe you are just unlucky
If your game had the void worms chosen as the mid game crisis, there is a risk that it may not be trigged as you AI empires are getting a difficulty advantage bonus to everything they do, and they are using it to eradicate all the nests before mid game starts.
