Toppopia is correct, Frozen planets can be used 3 times, ice asteroids once each. Your ice mining module should deconstruct automatically when all available ice is used up, although I've had bugs where it deconstructed early at times.
Each ice mining module can support one planet upgrading at a time. So if you have 3 different stations with ice mining modules, you can expand 3 different planets at a time.
As for the frozen planets, when they are used up, they turn into barren planets, possibly 3 sizes smaller? I haven't checked on the size part. Ice asteroids simply disappear leaving a normal asteroid belt with no asteroids.
