Are these uncolonized worlds inside your borders? The artifact might require a world within your territory. Or, possibly, the artifact only works on planets you've already colonized? (that last one isn't likely, I'm making guesses here)
Also, what do you mean they don't have a designation? You shouldn't need anything, just colonize the planet, use the relic immediately after, then use the Nu-Baol Life Seeding decision.
As for terraforming in general, you need certain tech to be able to terraform inhabited planets. If you want to terraform into a Gaia World normally, you need the World Shaper ascension perk.
Edit: Just saw your comment. You aren't supposed to be able to use the decision on worlds that aren't colonized.
I have only ever been able to get Eucmetropinan worlds. I play Humans, starting on Earth. Earth STARTS as a Eucmetropinan world. So as I understand you, Earth can never be improved. Every other world I colonize starts off as a Eucmetropinan world as well. I thought making those worlds a Generator world, or a Agrarian World, et would change that, but the "X - Not on an Eucmetropinan world" disclaimer keeps coming up on the Decision option.
With things as they are, unless someone tells me something different, I see absolutely no use for this artifact.
earth shouldnt be starting as an ecumenopolis unless you have mods. likewise, planets should not become ecumenopoli without player input. they're city-planets, a planet doesnt just randomly become a giant city.
figure out what mod you have that's causing this and remove it.
(alternatively, there's severe miscommunication happening in this thread. posting a screenshot of these planets may help especially one thats uncolonized and the same one after colonization)

Eucmetropinan isn't a thing. So if you're not talking about ecumenopolis then I'd say mods.
