I never realized they could die of natural causes up until I read this thread.
I want to go "Woo!" in reference to the time Bender became mortal but it wouldn't be a good reference unless I explained it...

Except for a chosen one ruler as long as their empire survives and they don't do the End of the Cycle.
It's the same regardless of their age. Every 10 years the game rolls a chance for a mechanical leader to die if you have them, and it's an empire-wide roll. So what OP is describing should not be happening, and is likely a result of the new patch ♥♥♥♥♥♥♥ with old saves.
Losing to a fallen empire can have them execute your leader?
