Chosen One selection
Three random leaders are selected. They are assigned high, medium or low chances to succeed at becoming the Chosen One. The player picks one leader with chances given for success, failure and critical failure respectively:
High - 70/20/10
Medium - 50/30/20
Low - 30/40/30
Success: leader acquires the trait "The Chosen One" and loses the "Psychic" trait, repetition of option blocked.
Failure: leader dies
Critical Failure: leader dies, shroud incursion.
Note that the Chosen One trait disappears once the ruler goes back to the leader pool, like if someone else is elected ruler in Democratic or Oligarchic governments. The Psychic trait is reinstated in its place and the leader becomes mortal again.

If the Chosen One becomes a ruler in normal means, i.e. an election, or you make your ruler a Chosen One, then nothing special happens.
However, there's a special random event for non-imperial empires with a Chosen One leader (not necessarily a ruler), which will make that leader a God-Emperor, change the ethics towards Authoritarian, and change civics to Aristocratic Elite, Philosopher King and Imperial Cult, which will also change the government to Divine Empire
As for my play throu after shroud event i did get chosen one ,as my empire was fanatic spiritualist/xenophobe i did get fanatic spiritualist/authoritarian with Philosopher king.
Afaik it changes all your civics, no idea if it swaps out civics that otherwise couldn't be, but I'm almost certain that it got rid of life-seeded for me.

3) I dont know if you can choose Imperial Cult as a FP. I have a Chosen one with no Imperial Cult civic as I am Inward Perfection
4) Chosen One doesnt change anything, its a perk.

There is an event I think that has to do with aliens that are your spiritual ancestors and you can choose an end result that turns you into a God Emperor or something along those lines but that has nothing to do with the Psionics/Utopia
But yeah, I might just do normal spirtualist guy without the genocide
Yeah, fanatic xenophobe is "I wouldn't care if all filthy aliens died writhing and screaming in a dumpster fire".
Fanatic Purifier is "let's construct huge mobile dumpsters and throw all aliens in it writhing and screaming as we grill smores and crack open a beer".
There's a bit of a difference there. :D
We're talking about an event that fires some time after you gain your Chosen One. A pop-up will inform you that some members of your species have taken a special shine to your Chosen One and think that he is some sort of higher being.
They have started a church of some sort and want to declare the Chosen One the new ruler.
You have two options:
Option A) All hail God Emperor Neo (makes Chosen One your immortal leader, changes to imperial and above mentioned civics)
Option B) Sure our Chosen One is a great guy, but a god? Hardly. Nothing happens.
Strange I never had this happen before. Is this a new update? I had a chosen one in 2.0 and neither of those options happened over a hundred years with that immortal chosen one as Celestial Empress.
Also I remember a mod that let you establish a ruler cult but I almost only play vanilla so I am not certain.
Might be new, actually. ^^
No mods here either, playing vanilla ironman so I'm very much certain that event isn't doctored in.
I think you can't actually have the Chosen One already be the leader, because then the event would make little to no sense.
I've had a Democracy when it popped for me and didn't choose my leader as Chosen One because I'd have just lost him/her after someone else was voted head honcho. Chose my admiral instead.
I'm pretty sure that's one of the requirements.
