Always dying (hp decrease)

Why does my health slowly decreasing every few seconds till i die? Is there a radiation in this game or what?

Nope, it's hunger. You need to eat to increase you HP or, with proper food, sleep. If you're following the Tutorial, you hit a point where you make a food grower (or Organic Grower) inside your base. Once you have that, you turn those Organics into Food (I don't remember if it's done at the Workstation or in Inventory). As you progress, you'll be able to make food supplies that provide larger quantity of food per Item. Once you build the Greenhouse, you'll have plenty of food available, but don't turn it ALL into Food, because you'll need the Organics for Teraformation.
