Foxiest of the Hounds
What does it mean no alarms? That means enemies cannot be in an ALARMED state? What about the cameras? What constitutes this? Thanks!

Never really tried it myself, but to my knowledge the game mechanics are like this: guard spots you, he notifies other guards. Often one of them sets off the alarm. The camera sweeps, when spotting you, it gives an unique sound, and if some guard happens to watch the monitor of the camera, he sounds the alarm. I think it is best to stealthily hack all security hubs, disabling the cameras. (You can click on the monitor which is active, changes it from scanning to inactive.) Look for alarm panels, try to hack them while nobody is around, disable them. In some locations look for cameras on walkway above you, they can spot you. It is tedious, but people have done it. I am not patient enough to do this all the time, but it saved my ass, when i lacked firepower.

You must avoid being seen at all, neither by camera nor guards.

I guess you don't have to set off any alarm, other words Red State. When they are in Yellow, Warned i think it didn't count, or would be very impossible to achieve, as only an opening door can warn a near guard, and many are stationary in their guard routes. So would be unfair by the game. I think it tell only the red state, by guards, cameras and turrets.

no. you can be seen by guards and cameras, but for short period.

Sounds reasonable, might try to go for a run. Mostly i play a mixed style and have fun in any "mode". What i really plan to do is a "I want to FU everything & get everyone against me" run.
I got"Ghost" fairly easily, some time ago, did it not on purpose. BTW, as soon as i open a door and hear a guard "Whos there" or the like, i retreat and re-plan my approach, you are right, did not trigger an alarm necessarily. I guess i figure it all out by myself someday. This game amazes me, because i can experiment with all kinds of stuff.

But it can be achieved at any difficulties or only in Deus Ex? because i was goin' to replay it in Rambo style in Deus Ex, but i don't want to replay it again on hard to achieve that.

Any. Best to play Pacificst and Foxiest on Easy :)

Alarm = MOST of the enemies in the area signaled as red arrows. E.G. spotted by a guard whom yells to others, spotted by a camera which makes the guards go on alarmed status.

Manually/laser triggered alarms system don't count, provided that they don't make the guards go red for other reasons.

But for example, if you are sneaking behind a patrol and he suddendly turns his back to you, going in battle stance and turning red, knocking him out in time will not harm your chances to get Foxiest (with "in time" I mean early enough not to make his comrades in the area turn red). Regardless of this, I still suggest you to reload every single time you get a red arrow.

Remember that the soldiers who ambush you and Farida on your second visit to Hengsha do count for Foxiest of the Hound, whereas any insane NPC from Panchea do not.

no, it is best to combine these with Legend achievement, on hard difficulty.

Those soldiers ambushing you only count for Pacifist, and Foxiest is safe, there's no alarm panels around.

Wrong. Failed hacks, alarms panel and laser grids are fine as long as you do not get spotted afterwards of before. Got a few failed hacks, an alarm panel activated because of an unconscious body found and accidentaly got caught by lasers in TYM but I still got Foxiest at the end.

I also beg to differ about the ambush, I got every smooth operator and all the ghost bonuses except during that sequence, and didn't get foxiest at the end. Then I reloaded, did that section properly, continued the game as previously done and got the achievement. And it's not just me, you can try googling "foxiest of the hounds helicopter".

I got this achievement, Pacifist, and hard mode in the same playthrough.
The things I remember doing is I never let a guard hit a Green Panel, I never let a camera spot me for too long, I never got locked out a hack, and I never hit a laser.
Some of those things may not have been necessary for Foxiest.
I was okay with turrets shooting at me, as well as going into firefights.
I can attribute to saving Malik.
Of course, all I used were the stun gun, tranq, gas grenades and takedowns.

My method of approach was I either took out all the guards, or I just went through the place without any of them going red.
If you punch a wall, make sure you take out patrolling guards. Took me a while to figure out why an alarm was going off.
Near the end of the game, when you have to save a bunch of scientists, make sure you complete the objective of shutting down the jammer.
Apparently, not doing that can void the achievement.

Pacifist it's a different achivement, and involve to don't kill anybody. Foxies of the Hound require that you don't set off any alarm, but you can stun or kill everyone you like.

Basically then, alarms can only be sounded indoors right? In open environments, there are no alarms, are there? And even indoors, not all places have alarm panels.

Would be good to know FOR SURE what can and cannot be done. What a waste to play again and after 40 hours see that you didn't get what you set out to get.

Just try to go for ghost and smooth operator in every possible section, that's the only way you can be sure you're gonna get it.

Save often and if you're unsure, quickload. I mind replay a little section instead of the whole game.

Guys, for example before entering the Harvester's Hideout, there are guards. However, there are no alarms there. Can I go guns blazing in such circumstances and still get the achievement?
