patch mod
are there any mods that patch the game into a playable state, without modifying anything custom?

The only unofficially patched map is c2a5c, as scene in this comparison here:

Unfortunately there isn't a unofficial patch mod available, but here is the c2a4c unofficial fix here:

I wish to see a similar fix of map leaks in c2a4c, but nobody had examined that map for decompiling purposes in Hammer Editor.
