u have to play the 3th day, and u will find a coupon close to the room with doors and stairs. It will allow you to buy the crossbar for 1000 $

After you buy the crowbar and start a new day, got to the door with the wooden boards nailed on it and use it with the right mouse button, go upstairs, take the voodoo doll and throw it in the disposal, then just play the game as usual, you'll see it at the end of the game

The doll reappear at the office if you go take it the day you get the crowbar tho.
