Are there always enough keys to open every door in dungeons?
There's one specific generation of the Catacombs dungeons that results in 2 (optional) locked doors the both lead to treasure chests, but I only ever see 1 key for them.

Wondering if I've just been missing it somewhere or if it's actually not possible to open both doors.

There are always enough keys to open every door. In some layouts, a key might be hidden in a place that isn't completely walled off, just takes a little platforming.

Thanks, I found the one I was missing the jump into the black space over the iron gate . That one was a lot more obtuse than all the other keys.

in one of the other variants, there's extra chests hidden in a similar fashion.

There's more places like that so do stay curious as you adventure around. There's hidden places that aren't associated with a key at all so you might miss them if you rely only on seeing a locked door to tip you off.

I believe there's only one instance this isn't the case: a variation of Catacombs where you're intended to go through the nearest locked door and go through a couple more rooms to get another key and get teleported back to the start for the last locked door, but you could technically just go back to the start and skip the middle section.

This variation is the one with the platforming to a lever to enter the dungeon proper.

But that's not technically "it doesn't have enough keys," just "one of the keys is behind a locked door, so if you go the other way, you skip half the dungeon"

Which, if you're just trying to farm the boss fight for drops, can be convenient

Under the criteria of "enough keys to open every door in the dungeon", meaning you COULD lock yourself out of that middle portion, it's the only known exception where you are able to use a key in such a way to lock yourself out of getting another key, but not locking yourself out of completing the dungeon.
