What do I do exactly?
Stack means you play in a premade party instead of solo with randoms. Also, use pings and voice comms to call out flanks and whatnot.
Once you get to higher level play such as GM, you can vastly benefit from reviewing your own vods from the enemy perspective. See just how silly you look when you're out of position, how open you are when you aren't peeking corners, the poorly timed ultimates or panic ults. You are making more mistakes than you think in Diamond and GM that are quite obvious once you see it from someone else's view.
Also analyze your losses and the type of team comps the enemy is running. Is there a particular comp or map that you struggle with more than others? I realized that, as a strategist, I struggled against the dive more than others in my elo. I became more aggressive and decided to take on duels until I learned how to win them or at least chase away the flanker.
Lastly, you mentioned playing vanguard. This is a central role for controlling the flow of the game. Do you use your mic and tell your team when you're being aggressive vs more passive? In solo queue, they won't know your ebb and flow which can put them out of position if you aren't communicating.
You're a stones throw away from GM, hang in there and you'll get it!
This all stems from players not wanting to accept that it's their fault they're losing because people want to have an ego. Your prior point about losing 2 and getting off can just be chalked up to a mental reset because losing 2 games is frustrating.
