They're laying in various setpiece locations. The final key that you actually need is acquired via following the story and hunting down the doctor that stole it from you. His location is revealed via completing one of two competing quests or by murdering a specific individual.
Also, you know the eatern Silent Forest entrance? There were TWO locked doors. I used the code that I got (3333) on both doors, but the only one that opened was a back door that lead me out of the house in a loop, like wtf. I've been EVERYWHERE now and collected EVERYTHING. Killed everything except the second deer. Is my game glitched, or where do I find another code exactly? What house/area and what location?
Now (aside from the metal door underground for finishing the game) I just have the ACTUAL exit to the silent forest lock, the wedding area lock and the lock at the top of the first chapter map, with no direction anywhere, even after reading through my journal stuff. What do I do?

That means my game IS glitched! The door to silent forest is one I cannot open with the code, and the code opened a different door that was a back entrance, wtf.
Oh, but one thing if you fix up your game; some text can be awfully hard to read. I can't remember exactly when, but I know I struggled with a code somewhere because I mistook a 7 for a 9, or something. That really doesnt matter here, if you managed to unlock one of the doors with the same code, but.. eh
