There's several endings you can get, but I'll assume you refer to the 'true' ending:
Once the epilogue begins, enter the large building.
1. First, check the postbox that has a postcard.
2.Then, go up the hallway where the homeless man is laying on the floor, try to open the metal door on the right, it won't open. When you walk away, a woman will open the metal door and tell you to be quiet. Walk up to the metal door and look at the baby.
3. Next, take the stairway to the basement, it's the stairway next to the entrance.
4. Walk all the way through the basement until you reach the storage room, look at the pot of jam and change the frequencies on the radio until it breaks and someone tells you to go sleep.
5. Go up the stairs to your appartment.
6. Once you're in your appartment, take off your coat. You'll notice roots coming out of the walls behind several pieces of furniture. Drag the couch and the television and examine the roots behind them.
7. Go to the kitchen and take the Large Screwdriver out of the kitchen kabinet.
8. Remove the floorboards, starting in the living room and going to the bedroom (the key to the bedroom is in one of the wardrobes in the living room).
9. Examine the bed, do not go to sleep, look under the bed, then move it.
10. Drag the bed out of the way, remove the floorboards and enter the hole.

Once you've entered the hole, you'll wake up in a very spoiler-heavy area. My advice is to explore it yourself and make your own decisions. But if you really don't know what to do:
When you touch the being, pull your hand away from it as soon as possible. Go to the left and you'll find a man clutching a flamethrower (Maciek), take the flamethrower from him and burn down everything. You can try to escape, you might even get pretty far, but you'll eventually die. That's the best ending.

thx you, but you forgot about tower

The Radio Tower doesn't seem to actually change the epilogue in any way, it's just a much harder way to get out that doesn't require burning the Talking Tree. If you manage to kill or run past the chompers and get out, you'll find yourself in the Epilogue as well.
