Mad Island is an open world survival craft, similar to games like "Don't Starve", "Ark Survival Evolved" and "Palworld". A little less similar to games like "Minecraft", "Terraria" and "Starbound".
I do not know of any good lewd games in this genre, but I heard "The Forest" is similar in bloodthirstiness. And "Rimworld" provides similar ability to be evil, but is an indirect-control small settlement simulator (and also much less graphic).

Conan Exiles kinda has some (not lewd) mechanics i.e. pve base building, exploring, slave collecting
