Fun fact, the indoor section is actually the same area as the surface! It's just really far below the map. It takes 11 stairs at max height to reach the ocean floor, but you'll need some clever tricks to travel between these realms.
Once you get close enough to the surface, NPC's function as normal. You can build a little society down there~ Just build up, let your follower down and then see if they wander to the nearest door or move around your territory (set it above 0).
Of course, drag houses from outside into the interior area and it'll create another interior below for NPC's :) don't get lost~
So extension could be made somewhat functional although it seems like they still somehow respect "territory" set and fail to see beds futher away from entrance based on location and territory set on ground level.

Ya sparked my curiosity so I went to observing.
NPC's warp into bed around 5~6 seconds. It didn't matter where I moved the bed on my second floor - above the entrance; middle; back, they would always warp after the set time. My bed placement is inefficient, so the ground-floor NPCs were also forced to execute this behavior. Since it takes about 5 seconds to walk to the back wall, it looks like it's an illusionary wall that transports NPCs to beds!
That's why I mentioned ya gotta get creative. While swimming, zoom your camera and drag it down with your mousewheel. Since the POV is at an angle, we'll dip underwater. From here, you can place anything you want and it can extend above the ocean. I placed stairs all the way up and built a mini-base under the mountain!
To get back down, you can either use bed-warp or place a house on the surface to take you back to the bottom. Perhaps the most efficient way would be to use the interior-level as a warp-zone. If you place the houses above just right so they're all offset on their X axis by a smidge and they're all exactly above each other, you should be able to line up the entrances so that they're all accessible from the merged single interior.
