List of how to get to the 6 secret levels

lvl03 (a3) & lvl04 (a4) goes to (as)
lvl05 (b1) & lvl08 (b4) goes to (bs)
lvl12 (c4) goes to (cs)
lvl14 (d2) goes to (ds)
lvl17 (e1) & lvl20 (e4) goes to (es)
lvl21 (f1) & lvl24 (f4) goes to (fs)

For the 3 secret levels that can be entered from 2 different maps, do items on the secret levels respawn when you visit it from another the other map? Such as you first visit (as) from lvl03, do they respawn when visited from lvl04?

For those secret level that have multiple entrances, I think it all depends on whether or not you destroyed the reactor for those levels. I know I've tried to re-enter a secret level after destroying its reacter, and the game wouldn't let me.

No, nothing respawns. If you destroy the secret level you can't go back at all. If you just leave and come back from another entrance later in the game, whatever you already took is still gonna have been taken.
