Chapter 1 - Spoilers!!!!

Hi guys and gals, I'm looking for some help with Chapter 1 of this brilliant game.

I've gone through it several times now and each time I seem to miss out on 4 or 5 quests and about 5 of the hidden items/areas. It's beginning to bug me so I would be really grateful if you could all help me fill in the blanks.

I'm not after locations for where to find the items/areas beyond broad hints, I'd just like to know what I'm missing. Thanks

Items/Areas I've found so far.

Secret area in subterranean incursion.
Da Vinci's blueprints.
Thor's Hammer
Wilhelm Tell's Crossbow
Hidden Prison Section
Dr Jekyll's Tincture
Mushroom Expert certificate
Wood Carver's Treasure (Don't know where or what it is but I had it, lol)
Scarecrow's treasure
Don Qijote's lance.
Goldfish
I've summoned the giants with the gong
Proved Woody is a ghoul
Got Dungeonkeeper's armour
Looted the dead young hunter
Found the secret area in invasion of the rat-things
Found the rare ore
Looted the grey wizard
Found the dungeonkeeper's amulet.
Got all 3 moonstones
Found spiritkeeper's relic.
Got the masterfully botched armour
Proved the yeti's existence
Defeated the Black Knight (no idea where he was but I killed him)
and I've got the rock from the talking stones of Perun.

The black knight is a little chess piece that you talk to.

The only secret I have that you don't is finding the Afterlife Ring (it's right after Invasion of the Rat-Things in my list).

Ah, thanks, I know who the Black Knight is now, he's the guy from Monty Python and the Holy Grail.
I'll keep an eye out for the Afterlife Ring

Here are two more:

Evicted the domovoy from the minecart (Cold Caverns)
Saved the bomb expert (Thunderhead Mt)

The Afterlife Ring is also in Cold Caverns.

One thing I'm confused about is Thor's Hammer. I've gotten the hammer from the Resistance mission in the two games I've played, but I've never gotten credit for the secret. Do you know what you did to get it?

I think I may have been mistaken about Thor's Hammer getting a mention as a hidden content for Chapter 1. Thanks for the extra info, I'll be starting another run through in a bit and will make sure to get them.
With luck that will just leave me with the missing quests to figure out.

It's the chest near the body that you need to loot to activate this one
