You sacrifice 25% HP in the Blood Altar, and it can be lowered to 12.5% with Skill Tree perk (there is one that lower Blood Altar cost by 50%). Then the upgrades feel almost free at cost of around 30-40 max HP.
You can find at least one weapon in each biome that is at the same level as the biome. The game always invite you to change weapon.
-- level 1 -- Village, Mines, Huns Camp
-- level 2 -- Aqueduct, Academy
-- level 3 -- Tower, Temple (+ bosses after level 2)
-- level 4 -- Garden, District
-- level 5 -- Palace* (+bosses after level 4)
I bolded biomes that include Blood Altar -- the Altars always upgrade weapon to level of biome + 1, Then, for example in Village you will get level 2 weapon, in Tower you will get level 4 weapon (and it's the most worth upgrade)
*Palace and one more biome should be added with 1.0 according to roadmap
All weapons scale the same in DPS and it is around
75 -> 100 -> 125 -> 175 -> 250
As you can see, the scaling is almost linear, and it's always worth to use Blood Altar in Tower to get level 4 weapon that has more damage lift.
Currently, you can not use level 5 weapons in action, because you can only get them after level 4 bosses, and there is nothing after that, yet.
