Gear shifter and knobs
I think it would be cool if there was a Gear shiftier in the cabin to make it more realistic for these who are into driving a manual and can have different knobs on the gear shiftier

Since nothing in the cab is interactive with the mouse and you won't see the shifter normally during driving (you should be able to shift without looking at it) it would do pretty much nothing.

I know I may get some flack for this but every single transmission is the game is an automated manual or fully automatic in the case of the Allison transmissions.

Hence no need for a gear shift to be modeled. If SCS has not modeled one by now then they probably will not be coming.

Some mods however do have these modeled, and are customizable.

Not completely - split gears you might need to feel (am I in 13 and can split down to 12 - or am I already in 12 and need to physically shift down while splitting up into 11). On a mountain this can be the decision between getting up and stalling. A replacement would be an animated shifter where you could have a short look to where the switch is flicked.
I really would like to drive the 9900i in manual - but as it does not have a gear indicator (to be more specific it has nothing, and even that only once a week) this is close to impossible and I had to return to the LoneStar.

VR it would actually be quite helpful..working with the same H shifter for everything, would be helpful to have the visual aid to know "okay those notches are all empty..." and as the game is really kind of all about that kind of simulation presence... yea.. i agree... would be cool.. kinda feels missing to me. just to have the model there and moving when you shift...

If you are willing to use a mod, this one here adds a gear indicator to the LCD on the dashboard:
