How do I play with my friend, do i have to install anything?

Go to the SCS Blog and look up "CONVOY" and it will tell you everything you need to know.
Also, there are videos on YT that are informative.

Convoy mode is what SCS Calls their multiplayer. It is built into the game, and you dont need to install anything else.

You need to know your friends steam download region and apply that to your steam settings to see all sessions within that region.
