18 for example would be when using a shift knob:
1L, 1H, 2L, 2H...get to 4th and then you flip up the range selector and have 5L, 5H, 6L, 6H up to 8L, 8H.
You will see 9, 10, 12, 15, 16, 18 etc. only when driving an automatic in game.

If you just want to use the splitter you should optimally only use a gearbox with 12 gears or less otherwise if i recall correctly you end up having to press the splitter 3 times to find what gear you want which gets confusing.

If in game settings you choose your layout as Eaton you will get the realistic 1LH, 2LH... if you set it to "Range + Splitter" you will get 1, 2, 3, 4...
Same for ETS2, choose any brand name gear box to get the HL pattern. Nothing to do with the truck brand.
As I play both games, and have every single truck available in each game, daily drive a different one, and do a lot of "quick jobs".
And getting older to remember every single layout, decided to use "Range and Splitter", so every time each gear is in the same position.
For the Low range, usually 1 to 5 (for 12 speeds), 1 to 7 (for 16 speeds) and 1 to 9 (for 18 speeds) I almost never Split the gear, ie. normal loaded will go 3, 5, 7.
And in the High range will split most of them, so 9, 10, 11, 12...
Also in my opinion is easier to use the advanced splitter Clutch and Accelerator, that means that you can split by just lifting the accelerator pedal or when you press the clutch.
that makes no sense...the splitter just gives you half steps between gears. The range function gives you access to the full range of gears from lowest to highest, but with no half steps in between. You can drive with range and no splitter, but not with splitter and no range.
the pattern would look like this on a G290 shifter

with the G290 you have to push the gear stick down to get reverse.
