in the real world the trailer wheels are moved to allow legal weight for the trailer axles. since ats doesnt have this level of realism yet, having the wheels forward will help with tight turns but you have to watch out for the rest of the trailer so you dont hit anything. Having the wheels to rear will give you the longest turning radius for the trailer. In the real world having a 53' trailer with the wheels to the rear while driving is against the law. The spread axle setup is most used for flatbed and reefer trailers because the distance between the axles allows for more weight to be loaded on the rear of the trailer.

but what is the spread setup for?

It is used for weight distribution, in the real world flatbed trailers can be set this way to allow a heavier payload than a regular wheel axle setup like a dry van trailer has. In the game it doesn't seem to have any effect on anything other then looking different and nice.

I'm just guessing here... but I *think* the game might actually take per axle weight into consideration in some way, I think it may (possibly) be a little more than a simple straight up 'it doesn't care'.

For example, another player (Robinicus) discovered that when he loaded cargo onto the 3-axle setup (where the front axle is a lift axle) that resulted in a high enough gross weight (77,000lbs I think it was), then attempting to lift the front axle got him a warning for being overweight and the axle refused to lift.

It *could* still be just a simplistic 'oh, you can't lift the axle if you're loaded' check, but it does at least show some hope that there's perhaps more going on (and... possibly more to come?).
