Report a translation problem
If running a 10 speed...you will need to assign a button for the range selector on your wheel or keyboard after getting to 5th gear. This is Shifter Toggle 1 in the controls.
If you are running a 13/18, in addition to the above you will need to assign a splitter too. However you will need to shift to the upper side after 4th gear. This is shifter toggle 2.
Do not put a check in shifter toggles use switch mode as you are not running a truck shifter like an Almars SKRS or an Eaton style shifter from Southern Trucking Gaming.
ETA: For simple terms...without a truck shifter...stick to a 6 speed transmission. You can then set each shifter position on the controls without the need to assign buttons on your keyboard/wheel for the range selector/splitter.

There are the following transmission in the game:
6 speed
7 speed
10 speed
13 speed
18 speed
Lonestar added a new transmission. A 12 speed for automatics.
6/7 speeds are also generally used for automatic transmissions. You can manually shift these like you do in a car however. That is why I recommended that.
Change the shifter layout to range. You are currently set to splitter by your description. You can then click on each shifter position 1, 2, etc and reverse. After you click each one, move your shifter to where you want first gear, second, third and so on.
How do I explain going 20mph at most in 5th gear? I still go 6mph in 1st gear, 8mph in 2nd gear. I have no idea what's limiting my speed. I vividly recall being able to at least do the speed limit in game. This is without trailer btw. Still can't even drive to my next job without my truck being a jack wagon.
Speed is going to be limited by max rpm...which is generally around 2000rpm.
Close out of the game
Navigate to Documents/American Truck Simulator/profiles\Your Profile Fodler(alphanumeric numbers)
Open this folder
delete the controls.sii file
Exit and start the game again. This will have set up your wheel and everything again.
Use Normal and Inverted if the axis is showing 100% on without the pedal being pushed.
I'm governed at 66 so 10 usually sits at 10rpm, aka low range for best fuel mileage.
