2: No. Difficulty can be changed at any time. There are no difficulty-related achievements.
3: No, not that I've seen.
Also, thank you for the quick reply. much appreciated.
im going to recheck just to see if i did not just press Z very fast going to test and edit the comment.
it did prompt for me now lol i guess i just pressed Z very fast since i play this since the beta, sorry.
Of course, this is probably one of the easier achievements to obtain. S ranking the arcade mode on the other hand made me gave up entirely.
Thought this was werid and made a second character and the same thing happened, straight into story mode, no difficulty prompt. It wasint until the following day, that i quickly made a custom character to see if that made a difference, that the difficulty prompt appeared. So it seems to be tied to (for me at least) first time bootup/installation of the game and first time character creation using the random function and a controller.
Add onto this that when i also made my first 2 characters, the game was not registering all of the keys on my keyboard (could not get steam overlay and screenshots to work). But i solved this issue by simply unplugging and then repluging my controller back in while the game was running.
As for the controller i use for the game, i use a wired Logitech f310 controller to be exact. Any game label's it as xbox, but it is a knock off of a playstation controller except it has a button that, in the event a game would normally force me to use the left joystick to move, i can press the button and it will switch it to d pad. All the button's on it seem to work so far for the game unlike some other controller's some people seem to be having issue's with which is nice.
But in anycase, all is good now.
