Arcade Mode Life Insurance
So far I have been able to get 3 levels of the life insurance policy for arcade mode which equals 75% gold earned during the run to be retained on death (very nice since my last run I had 42k gold saved up).

1st Level: build the bank and talk to the manager
2nd Level: spend 100k gold to upgrade
3rd Level: bring the manager a gold (?) Lood Egg
4th Level: this has not shown up for me yet? Anyone know how to unlock level 4?
