I've also had trouble with the trigger button locking up (LT) aswell. Funny you fixed the A button not working correctly. You know what I did? I used xpadder to get mouse left click on the button and just hovered my mouse over it. #povertystyle

Umm..it's still getting stuck and not working properly...

To recreate the problem:
Start a "Survival Game" (easiest mode to recreate the issue)
After starting the game, die / restart
On the next game try pressing A to use stun when the meter is charged.

Sooner then later it will simply "stop working" for no reason.
Note: All other solos will work, and the "space bar" will NOT activate the stun ability.

mines even gotten to the point where not only the A will stop working but so will X, Y, and B.
