Also @OP , i see u may have fixed it , I would still recommend doing the above as it wil greatly improve ur system health (fast boot is bad with 32GB ram and SSD , and SFC is build in error scan and repair for windows)

remember fast boot has 2 locations:
-your bios
-windows: power options > choose what the power buttons do > change settings that are currently unavailable > untick turn on fast start-up and I also like to disable Hibernate pish.
I'm on a 14900K + RTX 4090
So here's what works for me:
- Open Task Manager
- Start the game
- Immediately right click on "PathOfExileSteam" and enable Efficiency Mode
- Done
You might lose FPS, but this works EVERY TIME, not a single crash since I started doing this.The problem about the efficiency mode is that it's disabling automatically as soon as the game pulls more performance. Also tried it on Windows 11.
