I now have the same problem as you.
As you may know, PoE2 sometimes causes PC to freeze during loading screens.
The current solution is to turn off Engine Multithreading when loading new zones. This prevents the freeze.
I forgot to turn it off just now and my PC froze during the loading screen.
Now when I enter the game, my FPS is permanently stuck at 20-25 FPS and nothing I change in settings will make it go back to 60 FPS like before.
If your CPU goes to 100% the game is too much for your CPU and you will need to upgrade it.
If the CPU goes to 100% and the GPU utilisation is lower than 90%, then your CPU is bottlenecking the GPU.
If the CPU usage is low to mid, and your GPU utilisation is under 90% and the framerate is low, check if the GPU limitation is due to power limit being reached, or temperature which means the card is temperature throttling.
If the GPU is stuck permanently temperature throttling, check the fans.
Also check for the core frequency, vs the core frequency stated by the board manufacturer. If the frequency is really low, the GPU is throttling back to keep the thermals in check, to prevent the GPU core from cooking itself.
