Cold damage: Any cold attack, Cold spells, and Cold DOT. Just depends on your build.
Not that simple. If youre using a cold dot or cold attack spell damage is worthless. It all depends.
They stack with eachother as long as the spell you cast stays cold!
Attack does nothing if you are using spells.

as a general rule, the more general the term which describes the damage source, the better. but you're right it is more nuianced than that. my poison specs from my attack damage, ailment damage, poison magnitude, damage, physical damage, chaos damage, dot speed, dot duration. if i were to write out which is better than which in that scenario i'd need an entire diagram. i simplified it as best i could given the limitation of a single line
