GPU running 100% with hardstuck 50FPS, on a 4060, fixes?
My assumption was always that the game might need a patch to fix that?

I run a 4060 8GB with Ryzen 7600 but stuck where GPU running 99% but FPS is hardstuck around 50FPS and CPU goes around 20-30% only. I doubt 4060 bottlenecks POE2

These are what I done so far but to no avail:
On DX12
- Set High performance mode to the app
- Disable Fullscreen optimization
- GPU Driver updates
- Reinstall the game
- Vsync on/off, FPS cap on/off
- Graphics all lowest/highest
- DLSS on/off
- Tried the standalon version, same

Are there any more fixes I should try

Depends on resolution, in game quality settings, and refresh rate. Might be normal.

I had the same issue and tried out hellalot of things to fix this. I have a 4090 and what finally did the trick for me was to go windows control panel > display > Graphics and put both path of exile 2 exes on "your graphics card"(high performance) setting. Also went into nvidia control panel and selected poe2exe and put Opengl rendering GPU setting to my graphics card and power management mode to prefer maximum performance. Then i could play ez again on dx12 without issues. Best that helped is changing opengl rendering to my 4090. Guess for some reason game tries to render the game also through my mainboard graphic card for whatever reason. But anyway try that out and i hope it helps you too.

This game is buggy as hell:

Not advertising for the streamer it's just a live example of what causes lag and bad performance in certain cases. In this case a certain support gem.

Double check you have Engine Multithreading Enabled

try to disable triple buffering, it worked for me

It's ground effects, especially fire. This was a problem in poe1, but was eventually fixed. The question is why the hell it's back. As well as absence of predictive networking.

More updates from me:
- Have enabled the engine multithreading already through out trying each fixes
- Have tried disable triple buffering, no changes sadly

Try to run older gpu driver instead of most actual
For me game runs 60 fps at all times with similar setup and old Ryzen 5 2600X (Cpu runs like 30% max with multihreading enabled) and I get warning every time I start game to update my drivers

What version you use?

I've had a similar issue on launch day, removeing drivers and performing a clean install fixed it for me.
