You slot blasphemy like any other skill gem, click it in the inventory and put it in an available socket. At that point it's EQUIPPED, but not active. not neccessarily at least.
Now blasphemy is casting other inserted skills, particularily curses. So you need to have another gem, for example enfeeble. So place it (or another curse gem) in your inventory, click it, and drag it into an available slot IN THE BLASPHEMY SKILL.
You then need to have the required amount of spirit available (60 per curse I believe) in order to ACTIVATE it. With m&kb it's a simple rightclick on it, after which the buff will have a circling animation in the menu and will show up in your general hud when playing at the top or bottom.
Create a skill gem that is a curse (for instance Enfeeble, Flammability, Temporal Chains, Conductivity...)
Whatever you want that works with your build, and add it as a "support" for blasphemy.
