Cannot reach the Hammer of Kamasa

I was fighting the Forgemaster in the Molten Vault and died as I killed the boss.
I respawned and i cannot re-enter the arena and complete the quest.

Ctrl+click next time you waypoint into the area. That resets instances, so you can fight the boss again and get your loot.
