What the hell is killing me?

I understand that my character is squishy. I hate death to lag spikes, low resistances, my own clumsy hands making me run into projectiles or puddles. But what i do not understand is when i'm VERY CAREFULLY inch my way through some map or expedition and without a warning BAM YOU'RE DEAD.

Is there any combat log i don't know about? Today i've encountered a mob that shot time bombs around but it appears that it shoots them regardless of how close you are and guess what? Until you get close - you cant see them on the ground! No indication whatsoever. Ok, that may contribute to some of strange deaths but others i really do not understand.

Any kind of map basically. But expeditions are most painful because monsters get a lot of extra affixes and any invisible ♥♥♥♥♥ they throw at you from across the screen seem to oneshot

Yea there seems to be some/many problems with stuff either clipping, or getting a bit hidden by the ground textures or flora. In stuff related to jungles.
