The cheapest early access pack gives you 300 MTX coins. These coins can be used to buy extra stash space and other special stash tabs to make keeping your stash tidy a lot easier and quicker. You also need at least one premium stash tab to sell items.
So while the game is free and you can get a lot of time out of it without spending a dime, if you plan on playing for 150+ hours you'll likely want to spend a bit of cash to get some stash tabs.
As others have said, for your $30 / £24 you get 300 in game points, which you can use to buy stash tabs or cosmetics. If you end up playing the game a lot, you will want at least that many points worth of stash tabs.
A lot of people got free access due to the amount they had spent in PoE 1 too.
what if i passed away in a year... you do know nothing is guaranteed? this is why i partially made a purchase, on top of some other reasons.
