Has the Inscribed Ultimatum quest drop been fixed?

I've done the quest for the Inscribed Ultimatum for act 3 (Trials of Chaos) and killed the boss but then the token for the quest disappeared, is there anywhere to get it again, cause otherwise I'm soft locked. I can't progress :(

By "the drop," you mean the Inscribed Ultimatum token?

Given this is your first time doing the Chaos trial, you will keep the token as long as you didn't complete the trial, otherwise it's gone/spent and you can get another one from mobs drop.

In the same area as the boss for the token? Cause I've been in the same area, resetting the instance for a fair few hours now, and got no drop what so ever for it. The boss dropped the token for the quest but I picked it up and it disappeared and was not in my inventory or stash.

Is it perhaps in the Chaos Trial device?

No. I have not started it yet. I need the token for it. I killed the boss for the token. The token (quest green) dropped and I picked it up, however, it was not in my inventory or stash. And it's disappeared. I'm soft locked pretty much. I can't progress because I need that token to start the trial.

I thought so. It's just pretty annoying, because I'm seriously enjoying playing monk. But I can't progress :(
