I died on the fight against the Golden Colossus, but it died to DOT right after it killed me. When I resurrected at the checkpoint, The Hammer of Kamsa is available to pick up, and the boss is gone, but you can no longer pull the lever or ride the crane down to pick up the hammer or fight the boss.

There are three things you can do,

1. Create a new instance via Waypoint, it will reset and you have to rerun the map and fight the boss again.
   
2. Sorcerer Blink skill can reach it.
   
3. It's not really a solution, but unless this is your first time doing this quest, otherwise you've already unlocked the Reforge Bench and can be safely ignored in Cruel difficulty.
