Still have Sacrificial Heart and cant get to the altar, since now the portal leads to cruel
Anyone knows what to do? Got the heart and cant get back to the apex and altar, since the portal now only leady to cruel.

You can waypoint back to act 3.

You can waypoint to black chamber (Doriyani boss fight last zone) and exit it to return to the area with the altar.

When I go back to Act3 I see only the WPs there to the drowned city, not to the past.

I got it, when using the WP in cruel to go back, I can get to the past and the apex. Problem solved
When using the WPs from act3 I only get to drowned city.
