Not sure about POE 2, but for POE 1 you can choose how the scaling works in your player settings. Here is results of a quick search I did for clarification.

The Level Downscaling setting in Path of Exile adjusts your level to play with friends of different levels:
Level Downscaling enabled
Your level is adjusted to play with lower level friends, and they receive full experience points.
Level Downscaling disabled
You remain at your normal level, and lower level friends receive a penalty to their experience points.
If you're playing with higher level friends, you can allow them to create the party and adjust the Level Downscaling setting.
Playing in a group in Path of Exile makes the game more difficult because enemies gain extra health, but it also increases loot drops for the entire party.

Depends on which part of the game you are talking about. If it's the campaign, probably set levels per zone. Once you get to the Atlas Map, it scales iirc
Atlas maps are not scaled at all. Area lvls are strictly tied to map tiers. Like go try run t15 at lvl60...
Its also reason why reaching lvl 100 is so grindy, highest content is like lvl 83 or 84, and thats for uber elders. Maps are 81 or 82, so you get HUGE XP penalty past lvl 95.
This is only for POE 1 and no info yet if it's gonna be the same from what I gather.
Map tier is a scale though.
Map tier is static. It can be boosted only by voidstones, but theres no scaling.
I don't think you get it. Tier = Scale
The higher the tier, the harder it gets. The lower the Tier, the easier it gets.
Then on top of tier you have modifiers. Modifiers are not a scale, but rather a static change to the scale.
