Personally i spend the most on the skill effects. I am totally hyped to see how they will look in POE2. You can see a video of each effect in action, so if you don't want to shoot boring fireballs, buy cooler ones. The same goes for most skills. Different totem skin for your totems, etc.
All purely cosmetic.

Other than that, stash tabs for hoarding shiny loot. Just a Qol thing, no real advantage.
There is no P2W, even if you look very very carefully.
