Who cares? D1 and D2 had gender-locked classes. POE1 somewhat did the same, kinda.
Nobody really cares you see.
1. 99% don't gives a ♥♥♥♥ about genders and customization.
2. People prefer developers time spent on making gameplay more fun.
You see, the character customization is so deep, it needs a whole different application! :D

Realistically, how much development time would it really take? I'm not saying we need sliders for everything, but male and female model with some face, hair and markings options wouldn't take thousands of hours.
Redoing all armour sets for new body types. Months to years for a single person. Thats just the graphics. You want the female and male monk to make the exact same moves? No...well then lets change all animations, too. For all classes.
I guess we could come up with more things that need to be changed. The real changes are probably much deeper.
