Forlorn Mask bug - did not give item?

I purchased Forlorn Mask while carrying Celestial Contract. The game briefly showed the 'Defeat' screen before I res'd, but I did not receive a Legendary or Epic item as described. Confirmed in the game log, no additional item was received. Seems like a bug
