Achievment questions, hints?

Draw dark letters 3 times and Chaos Theory? My game bugged out when the main lady wanted to go tagging so maybe I do it there? As for the Theory ach, what are the correct answers, I did my best but still didn't get it so I am unsure how to find it out, listened to all radio broadcasts and called all numbers. Appreciate any help :)
