Exhaust must be directed through pipes out of the vehicle (can be through a hull pipe, standard pipe, or "exhaust" piece), and that area must have "cear flow", meaning no part of the ship is in front of it at any distance (think similar to detection gear. Even if it's 50m away, if it's in front, it's "blocked")
If you connected them to cylinders and nothing works i suppose you placed the engine block the other way around and then nothing is connected.
I forgot when, but after some update it really doesnt matter which end do you connect to the cylinder.
