I've been around the cave several times, lit all the torches and no mission was completed

its possible you didnt light all the torches.. i went through the caves 5 times with great effort, and after the fifth time... somehow discovered that I missed 1 torch the first 4 times.. and got the achievement finally

I have been in and counted, and i still need that completed as well. i counted 40 torches. all lit.

if your having trouble finding torches you can join a multiplayer lobby that has the achievement completed and get it that way. eg i just got all achievements so now i have an end game save so i could host it for you to get achievement

Four torches are in the 'Cultist' area on the columns, I needed to use a Feather potion to be able to jump high enough to light them. Hope that helps.

I have same problem. Missed 4 torches in boss room but it still not enough.

I did it. There are 8 torches in boss room. At least 2 of them are inaccesible if boss still alive.
