How is using titles cringe? I think OP is actually right, it's the little things that matter and elevate the experience. Sure, we know who they are because it is mentioned a million times but it wouldn't hurt to give them more more "epic" names/add titles.
For example, Isshin, the Sword Saint sounds a lot better than just Isshin.
It's the little things that make the game better... this person also said that "We already know who they are" which is a bit of a stupid argument because we knew that we are fighting Hismar, Ozma, Trokka, Skalpel, etc. so why bother having their names at all under their HP bar when we already knew who they are, you know lol.
As for your point about Isshin, there are actually more than one Isshin, so it makes sense to distinguish them by a title. The Sword Saint you fight is a kind of soul-demon thing summoned by Genichiro. And if you go another route, you fight Isshin Ashina - just the name, no titles. Genichiro also doesn't have a title, btw. Until he demonizes, to signify the difference. It also serves as a way to tell the player what moves he will do.
I don't remember it exactly because I played Sekiro like 5 years ago, but I have a feeling that Isshin dies when going for the Sword Saint ending and Genichiro just summons him back in his prime form. When going for the Isshin Ashina ending, you are fighting the old Isshin who is still alive at that point of the game (I think).
