How to achieve THE FIRST BERSERKER?

Hey there,
i did every bonus mission and collected every collection item and thus got a 3rd phase of Ozma whom i beated, but still didn't get that achievement - what else am i missing?

Did you pick "I want to know the truth" choice while speaking with Daphrona during the final mission. Even if you collected all the golden collectibles picking the wrong choice will lock you out of the first berserker ending.

Daphrona's dialogue offers 3 options and 2 of them will give you three phases final boss.

The only option that won't give you 3 phases is the revenge one and the first berserker ending is the "I must know the truth" one. The other one will give you 3 phases, but it's not the true ending, therefore you won't get the achievement.

You gotta collect all the golden pages, complete the two Blade Phantom missions and select "I want to know the truth"

that means you either didnt have the 15 golden items and/or did not complete the charon's chains and transcendent blade side missions before talking to dephrona. if you did, a third option would be there "I want to know the truth" which is the option you have to pick for true ending.
