What weapon are you playing with? What does your build look like? Have you tried using buff items? Have you watched a guide video? Have you considered summoning a spirit or switching to easy mode (in case you're still on normal)?

Please don't give up!! She has very few attack patterns. + there's no phase 2. You just need to memorize every attack of her. It may sounds ridiculous to you but it's quite easier than defeating previous bosses. Just try to guard or dodge the enemy without attack. You'll see what I'm saying soon.

Tanya? Just don't exchange hits with her, she has lifesteal on melee attacks, even if you brink guard, so just dodge instead, it's okay if it takes a while.
Be patient and do hit & run.

Skalpel is one of the easiest bosses in the game (assuming this thread is about him). Phase 1 you just sit in front of his face so all he does is the same few easily reflectable sword attacks. Phase 2 you just run. I've legitimately never hit him in phase 2.
