What's there to do after finishing the first playthrough?
Is there NG+?

There are three endings, and there is NG+. NG+ introduces new item rarity and stronger enemies. I believe it only goes to NG++.

There's NG+ and NG++. NG+ unlocks transmog, expands your skill tree slightly, and a new rarity tier for gear. NG++ unlocks a second new rarity tier for gear.

The idea is to enjoy the game again with a full skill tree and a more refined build. Rarity tiers for gear unlock new fixed affixes for equipment, some of which are unique skills, opening up new build possibilities and power levels.

Since the 3 weapon types play very differently and you probably don't want to respec all the time, for me it's playing once through with each weapon. I just switched from spear to greatsword on NG+ and it's like a different game, getting my ass kicked again ;-)
