How can I change the display output/screen?

you can press WIN+P to select your secondary screen, after you are done playing then press it again to go back to extend. probably not exactly what you are wanting but its a workaround.

That should be done in the graphics driver usually, not the game

The real answer is to press down shift+windows key or shift+start on keyboard and press the right directional arrow until it swaps to the screen you want the game on.

No need to go to settings menu.

But I can't change the res to 4K. It keeps the 1440p (or lower) of my main monitor. Second screen is a 4K TV.

You should be able to change the resolution once you move over the game to the second screen and make sure it is in full screen mode.
