Missing Disk Fragment

Aldevir's Disk Fragment which is required to actualy get the achievement for finishing all bonus missions dissapeared from my inventory. I thought i had missed it, but i went back to the Aldevir Citadel Blade Nexus and it wasn't there anymore. Did this happen to anybody else?

Happening to me right now. I feel Like I would've grabbed that one too it's right in the open and it's not in my inventory

Figured mine out . I do have all 3 , the first one I thought was missing is just a lower rarity, I have all 3. I talked to the soul stone girl NPC and she unlocked the side mission

Yeah i figured that out eventualy. Thanks for the answer tho, might help others.
